Remote Companion for Chronic Pain Monitoring

About the role

CoachFlow is seeking compassionate individuals to serve as remote Companions for patients enrolled in a chronic‑pain monitoring program. As the human voice patients look forward to hearing, you will build rapport, collect health data, and ensure clinical concerns reach the care team.

Key responsibilities

• Make scheduled weekly check‑in calls with a small, consistent panel of patients.
• Collect patient‑reported pain and mood data using validated instruments.
• Monitor the daily platform queue for engagement signals, missed sessions, and flagged scores.
• Follow scripted protocols to answer common questions and support patient outreach.
• Identify and route clinical concerns to the Nurse Practitioner team.
• Document each call accurately in the CoachFlow platform.
• Build genuine, ongoing rapport so patients recognize your name and voice.

Required profile

• Strong communicator with a warm, steady phone tone.
• Comfort with structured workflows and platform‑based documentation.
• Background in care coordination, medical assistance, community health, peer support, or hospitality is a plus.
• Bilingual (Spanish/English) is a plus.
• Reliable home internet and a quiet workspace for patient calls.
